Kindergarten registration for the 2003-04 school year begins
April 28. Registration packets may be picked up at the District
Enrollment Center, 15600 Concord Circle or at any elementary
school, beginning April 1. Completed applications must be returned
to the District Enrollment Center and will be accepted beginning
April 28. Student registrations will be processed when all
requirements are met. The following information is needed for
enrollment:

The following information is needed for enrollment:
•Birth Certificate or Passport confirming that the student was born on or before December 2, 1998
•Proof of Immunizations (California-yellow state immunization record)
•Health Physical (white CHDP form from a doctor’s office)
•Residency verified by one of the following:
•Copy of lease/rental agreement indicating the manager’s or owner’s name and phone number
•Utility Bill
•Driver’s License
•Escrow papers
