began March 15.
Kindergarten registration is under way throughout the Morgan Hill School District for the 2004-05 school year. Registration began March 15.
Packets may be picked up at the District Enrollment Center, 15600 Concord Circle, or at any elementary school.
Completed applications must be returned to the district Enrollment Center for processing. Student registrations will be processed when all requirements are met. Enrollment at your school of residence is on a first-come, first-served basis. Students currently enrolled in the district do not have to re-enroll.
Your registration is complete when the following documents are returned to the district enrollment center:
Birth certificate or passport: A copy of the original birth certificate, baptismal certificate or passport confirming that your student was born on or before Dec. 2, 1999.
Proof of Immunizations: A copy of immunization records with annotated physician signatures verifying dates of immunizations is required. Also, Santa Clara County requires that all new kindergarten students have documentation of a TB test given no more than 18 months before entering school. (All other students coming from out of Santa Clara County must show proof of a TB test no more than six months before entering the district.) This test must be documented with the following information: date given, date read, number of millimeter’s enduration, positive/negative.
Health physical: A physical exam is required for entrance into kindergarten and first grade and must take place not longer than 18 months prior to entrance in grade 1 or six months prior to entrance in kindergarten (must be dated March 1, 2004, later for kindergarten or March 1, 2004, or later for first grade) or proof of appointment presented at time of registration. (When planning for your child’s kindergarten physical examination, the appointment must be after March 1, 2004.)
Residency: A utility bill, valid driver’s license, escrow papers indicating the street address or a copy of your lease/rental agreement indicating the manager’s or owner’s name and phone number. Post Office Box addresses are not acceptable.
